At Harmony In Care, our clients with complex care needs are typically referred to us from an NHS Clinical Commissioning Group (CCG) or from an independent case management company. However, we also work with local authorities and directly with privately funded clients.

Clients receiving complex care have substantial and ongoing healthcare needs which can be the result of chronic illness, disabilities or following hospital treatment, which is why we are committed to providing them with:

For dementia clients who are reluctant to go to a nursing home, home care offers one-to-one, personalised care. This is safer, helps the dementia patient remain around familiar memories and familiar surroundings and leads to fewer hospital admissions than that of a dementia care home.
